Birth Certificates

Available for Missouri births after 1920 and can be printed in-office while you wait.

Available for Missouri deaths after 1980.

Fees

Birth Certificates: $15 each

Death Certificates: $14 for the first copy, $11 for each additional copy

Payment accepted by cash, credit card, check, or money order.

Processing

In-person requests are issued immediately.

Mail-in requests must be signed, notarized, include payment, and are processed the day they arrive.

Required Information

• Full name

• Date of birth or death

• Your relationship to the individual

• Purpose of the request

• For birth certificates: father’s name and mother’s maiden name ID is required for in-person requests; mail-in requests must be notarized.

Certificates are available immediately if you present the form in person. Certificates requested by mail are processed on the day of receipt.
