Computer downlink allows Jasper County Health Department access to vital records at Missouri Department of Health in Jefferson City, Missouri

 

Certified copies of Birth Certificates can be printed at our office while you wait. Certificates are available for any birth occurring in the State of Missouri after 1920.

 

Certified copies of Death Certificates can be printed for any person whose date of death is after 1980. If you need a Death Certificate for a death that occurred prior to 1980, please contact the State at:

Birth Certificates are $15.00 each. Death Certificates are $14.00 for the first copy, and $11.00 for each additional copy ordered at the same time. The Health Department accepts cash, credit card, personal check, and money orders.

Certificates are available immediately if you present the form in person. Certificates requested by mail are processed on the day of receipt.

To obtain a birth or death record, you will need:

Full name

Date of birth or death

Your relationship to the individual named onthe record

The purpose for the copies

For birth records, include Father's name and Mother's maiden name
